Enroll Your Child

We are excited to welcome you and your little to the SproutFive family. At SproutFive, we accept students from six weeks to five years old into our program. Before enrolling your child, we recommend you tour our program and fill out the required enrollment packet. When completing enrollment information, we will need an updated physical and shot record, a copy of your child’s birth certificate, and a copy of your photo ID.
